Almost definitely hitting error 131049 - Meta's sneaky frequency cap on marketing templates.
Even if your template's approved and has no variables, Meta quietly flags it on their end. If they've tagged it as "Marketing", they'll start silently blocking delivery to some users depending on how many promo messages that number's had recently. Approval means nothing here.
Check your API logs for 131049. if that's the code, request a re-categorisation to "Utility" in Meta Business Manager. Rewrite the message to sound transactional - something specific to that lead's action, not a generic "welcome" blast.
for what it's worth, retrying failed numbers after 24 hours recovers maybe 40-50% of them. the cap is per-user and temporary, but still annoying.
